The object of the study is patients with metastatic lesions of the brain. The purpose of the work is to increase local control of tumor growth and reduce the risk of developing post-radiation pseudoprogression and radionecrosis by performing stereotaxic radiosurgery at the preoperative stage. Research methods are immunological, pathomorphological and biochemical studies. Methods of radiosurgical treatment of patients with metastatic brain tumors at the preoperative stage with quality of life have been developed; identified indications and contraindications for radiosurgical treatment of the indicated group of patients; for the first time, prognostic factors of survival were identified depending on the features of preoperative stereotaxic radiosurgery, immunological and biochemical aspects, and pathomorphological characteristics of tumors. The introduction of the latest developments made it possible at the pre-radiosurgical and pre-operative stage to determine the degree of risk of neurological deficits, local recurrence, optimize treatment tactics and minimize post-radiation radionecrosis.
