The purpose of the III stage of research work is to develop an algorithm for choosing the method of surgical functioning neuralgia of the trigeminal nerve. Research methods: clinical and anamnestic, neurological, neuroophthalmic, otoneurological, radiological, neuroimaging, biochemical, and various methods of neurosurgical interventions for trigeminal neuralgia, statistical. Results. 47 patients with severe trigeminal neuralgia were conservatively treated. The main options for differentiated neurosurgical treatment of the main clinical forms of trigeminal neuralgia have been selected: blockade and laser neurotomy of the peripheral branches of the trigeminal nerve, balloon microcompression of the trigeminal node, stereotactic retrogasseral laser neurotomy of the sensitive root of the trigeminal nerve, microvascular decompression of the trigeminal nerve root. Surgical interventions were performed in 94 patients with the most severe and pharmacoresistant variants of trigeminal neuralgia. Acomparative analysis of the effectiveness of neurosurgical methods for the treatment of patients with severe forms of trigeminal neuralgia, considering the severity of pain before treatment, etio-pathogenetic factors of the disease, general somatic condition, concomitant pathology, age patients, the duration of the disease, previously applied methods of treatment.
