The aim of the work was to increase the effectiveness of treatment of patients with onychomycosis by optimizing methods of diagnosis, complex differential therapy, assessment of curability and clinical significance of concomitant pathology, presence of absolute and relative contraindications to systemic antifungal therapy. Methods: clinical, biochemical, immunoenzymatic, microbiological, molecular genetic, instrumental, statistical. A complex method of treatment of patients with onychomycosis with absolute contraindications to systemic antimycotics using topical antimycotics and agents has been developed, which allows to achieve clinical and mycological recovery in 72.7% of patients. A differentiated approach to the treatment of patients with onychomycosis with contraindications to systemic antimycotics is scientifically justified, which allowed to achieve clinical and mycological recovery in 90.8%. It has been proven that in patients with onychomycosis, it is necessary to monitor cytolysis enzymes, urea during long-term treatment with systemic antifungals and use hepatoprotectors. It was established that the use of infrared thermography confirms the violation of blood circulation, which justifies the appointment of vascular drugs and can be used to control temperature fields in the process of sanogenesis. The detection of specific Ig E makes it possible to assess allergy to certain mushroom allergens and to predict the development of allergic reactions. The algorithm for examining patients with onychomycosis after treatment has been improved in order to establish the viability of fungi and monitor recovery.
