The aims is to form randomized samples of people, develop a research protocol to determine radiation cerebral diagnostic criteria. To conduct neuropsychiatric, neurophysiological, neuropsychological and clinical radiological studies to determine the cerebral effects after exposure to ionizing radiation and their differences from chronic cerebrovascular pathology in non-irradiated individuals. Methods: clinical neuropsychiatric, psychodiagnostic, neuropsychological, clinical radiological, neurophysiological, statistical. Randomized samples were formed. Approaches to the study of differences in radiation-associated cerebral effects from chronic cerebrovascular pathology in non-irradiated individuals have been developed. In the Chernobyl clean-up workers signs of organic brain damage were found, with the predominant involvement of the cortico-limbic system of the left (dominant) hemisphere, a violation of information and cognitive processes of the brain against the background of functional hypofrontality. An excess of stress-related and somatoform disorders, mainly due to post-traumatic stress disorder, was found in the Chernobyl NPP workers who were at their workplaces under the conditions of occupation of the facility by the Russian military in 2022. Signs of hyperactivation of the cortico-limbic system, mainly of the right (subdominant) hemisphere, physiological hyperexcitability of the main sensory systems, functional hypofrontality, which is most likely a psychophysiological response to prolonged psychoemotional stress, were revealed.
