The aim of the study: to optimize the identification of risk factors for the development of obesity in patients to prevent its progression and develop methods of prevention and treatment.Research methods and equipment: biochemical, immunoenzymatic, instrumental, statistical.Results: The study of the levels and relationships of incretins (GLP-1), adipokines ( leptin and RBP4), neurotransmitters (tyrosine-tyrosine peptide) in patients with excess body weight with carbohydrate metabolism disorders was conducted. It was established that in patients with DM, the level of GLP1 is probably reduced compared to the control group of patients without carbohydrate metabolism disorders. GPP level did not depend on BMI, visceral fat level and HbA1c level. An increase in the level of leptin in obese patients was established, it had a positive correlation with BMI, insulin level and the level of total and visceral fat according to bioimpedance measurement. A decrease in leptin levels was noted in obese patients with newly diagnosed type 2 diabetes compared to a group of obese patients without impaired carbohydrate metabolism. A decrease in the circulating level of leptin and GLP-1 in obese patients with diabetes can lead to stimulation of appetite, weight gain and progression of the degree of decompensation of diabetes and insulin resistance. The level of the anorexigenic polypeptide hormone tyrosine-tyrosine peptide was also studied. The level of tyrosine-tyrosine peptide was probably lower in obese patients, it did not depend on the degree of compensation of violations of carbohydrate metabolism according to the level of HbA1c, indicators of insulin resistance.
