The purpose: to improve the immediate and long-term results of reconstructive surgery of the aortic root in patients of different age groups by developing a differential approach to patient selection and implementing new surgical techniques. Research methods: in order to determine the severity of the aortic defect, indications for surgical intervention and its effectiveness, general clinical, ultrasound and radiological research methods were used. Evaluation of the effectiveness of the use of the developed surgical tools was carried out in an experiment on a macropreparation using hydrodynamic testing of the autopericardial valve. Comparative analysis and assessment of the reliability of the obtained indicators was performed using statistical research methods. A protocol for differentiated assessment of AV was developed and implemented in order to choose the optimal reconstructive technique using two- and three-dimensional echocardiography, pre- and intraoperative assessment. Reconstructive techniques have been implemented and improved in children and adults with aortic disease. The immediate results of AV reconstruction in children and adults were evaluated. The experimental part was carried out: hemodynamic testing of autopericardial neo-valves of AV, cut out with the help of the developed toolkit, in the conditions of the in-vitro experiment. The valve was evaluated according to the developed and implemented protocol (two- and three-dimensional echocardiography, pre- and intraoperative evaluation). The use of domestically produced gauges and patterns during neocuspidization of AV was implemented (50 patients were operated on).
