It was found that T1DM was debuted in 179 (53.5%) of 334 (48.0%) DAAb-positive normoglycemic children during 20 years of observation. In individuals with a faster clinical manifestation of T1DM and a duration of the preclinical stage of the disease development of less than 3 years, the prevalence of elevated IA-2A, GADA and ZnT8 titers is found both in the early and in the final stages of the preclinical period of T1DM development. We studied the adaptive and innate immunity at the latent preclinical stage of the development of T1DM. A developed algorithm for preclinical diagnosis and development of T1DM in healthy normoglycemic children using the determination of different types of DAAb (GADA, IA-2A and ZnT8) was implemented in the clinic. It has been established that vitamin D3 takes an active part in modulating the functions of the immune system in the development of T1DM and has a protective effect on the risk of developing T1DM. The effect of using vitamin D3 in a dose of 1000 U/day in the predicted early preclinical stages of the T1DM development and in a dose of 2000 U/day in the late stages of the preclinical period of the TDM1 formation on the titers and levels of cytokines in the group of DAAb+ children was analyzed according to the combined algorithm for predicting the timing of the T1DM debut in DAAb+ children. It has been shown that inhibition of autoimmune aggression through the use of vitamin D3 at the stages of T1DM development can serve as a pathogenetic and protective therapeutic factor in the further disease development.
