The aim of the study is to carry out a systematic analysis of the structure of stable disability due to the most common neoncological and oncological diseases among the victims of the ChNPP accident - residents of the Kyiv region in the remote postaccidental period. The Study is performed in the design of retrospective research on the materials of medical expert cases of the Central Interagency Expert Commission of the Ministry of Health of Ukraine (during 2013-2020 41 407 expert medical cases were considered) with the use of a systemic analytical approach to the assessment of anamnestic, routine general clinical, biochemical, instrumental indicators, doses of radiation and evidence statistical analysis. The total number of victims of adults as of 01.01.2021 compare with 2008 decreased by 441 444 people, or 24.06 % (from 1 834 536 to 1 393 092 people). There is an increase in the proportion of persons with category 1st who have lost their ability to work due to diseases associated with the action of radiation exposure and other harmful factors as a result of the ChNPP accident up to 105 096 persons or 6.15 % of the total number of victims. The existing system of medical and social examination to establish a causal connection of diseases, disability and causes of death with the impact of the Chernobyl accident requires scientifically sound renewal and optimization, taking into account new data in the field of radiation medicine and changes in Chornobyl legislation. With the help of retrospective analysis, the study of the structure of medical expert cases during 2013-2020 was conducted and the frequency of consideration of medical expert cases regarding cancer (53.69 %) and cardiovascular disease (39.62 %).
