The purpose of the work: to improve the results of treatment of children with CPT by improving surgical techniques, developing fixators for surgical treatment, determining the optimal methods and terms of fixing the bones of the leg, factors that prevent refractures. Research methods: clinical, radiation, biomechanical, statistical, conceptual and logical modeling. Theoretical and practical results: clinical and radiological data on the peculiarities of the development of the affected limb were supplemented, a CT assessment of the volume and X-ray density of bone grafts after performing free and "wrapped" autoplasty with the use of various osteosynthesis techniques was carried out. The obtained results were showed a significant advantage of "wrapped" metal-bone autoplasty. Finite element models of CPT were created and various types of osteosynthesis were mathematically modeled. A comparative analysis of models of osteosynthesis in the case of CPT was carried out. It was determined that the formation of tibia and fibula synostosis with fixation by an intramedullary rod leads to a decrease in stress in the CPT area. An algorithm for the selection of surgical treatment of CPT has been developed, taking into account the CPT type and localization, the technique of bone autoplasty with titanium mesh and the use of various types of fixation. Novelty: an improved intramedullary telescopic fixator (ITF) for the treatment of children with CPT and the technique of "wrapped" autoplasty of the CPT area using periosteum, bone cortical-cancellous autografts and titanium mesh were developed and experimentally substantiated. Effectiveness of implementation: obtained new knowledge can be used to improve diagnosis, planning, surgical treatment of children with CPT. Field of use: medicine.
