The aim of the work is to study the activity of hemolytic processes in patients with diffuse large B-cell lymphoma depending on the degree of aggressiveness of the disease for the development of informative diagnostic criteria and justification of treatment tactics. Research methods are general clinical, immunohematological, biochemical, statistical. We examined 32 patients with diffuse large B-cell lymphoma (B-DLCL). Reference group consisted of 28 samples of venous blood from healthy individuals. Results. A low risk of developing autoimmune hemolysis in patients who don`t have anemia is established in the presence of anti-erythrocyte antibodies in a low titer and the absence of biochemical markers of hemolysis and reticulocytosis, and a high risk is established in the presence of autosensitization, one of the biochemical markers of hemolysis, moderate reticulocytosis. The aggressive course of the disease was accompanied by a decrease in osmotic resistance of erythrocytes, a complementary type of reaction, antibodies of different serological types. At the initial stage of the malignant process heat and/or cold antibodies were detected. An autoimmune reaction of a complementary type was observed at the advanced stage. The influence of BCL-2, BCL-6 and C-MYC expression on the development of autosensitization and stimulation of an increase of osmotically weak erythrocytes in the bloodstream has been established, which requires timely correction of therapy. Patients older 52 years require regular studies of hemolytic markers due to an increased tendency to develop autoimmunization. Medical and social effect. Medicine.
