The aim is to increase is to improve the efficiency of molecular genetics methods of tuberculosis diagnosis by developing TB diagnosis algorithm and determining the drug resistance (DR) of M. tuberculosis to antimycobacterial drugs. Methods – genetic, bacteriological, statistical, equipment – molecular genetic systems and phenotypic system BACTEC MGIT 960. The developed algorithm of pheno-genotypic diagnosis and monitoring of treatment of TB with DR using modern rapid methods, which are proposed to be used as an initial method for all cases of the disease with clinical suspicion of TB, as well as for the purpose of detection, differential diagnosis and accelerated determination of DR MBT to AMBD I and II lines. It has been determined that the MG test GeneXpert MTB/RIF Ultra should be used as an initial diagnostic test for all adults and children with signs and symptoms of pulmonary TB. It is suggested that the MG test Xpert MTB/XDR be used in the practice of TB of the 3rd level laboratories of Ukraine, and the method of determining the DR profile of MBT strains using the Xpert MTB/XDR test be implemented in the TB diagnostic algorithm. It has been established that it is advisable to carry out fTMS of MBT before AMBD of the 1-st line (Н, R, E, Z), before AMBD of the 2-nd line - before Lfx, Mfx (in two concentrations), Bdq, Lzd, Cfz, delamanid (Dlm), Am, and if necessary to Km, Cm, Pt/Eto. If the MBT is sensitive to R in the Xpert MTB/RIF Ultra test, fTMS should be performed before the 1-st line AMBD, and if the MBT is sensitive to R in the Xpert MTB/RIF Ultra test, the fTMS should be carried out to the 1st and 2-nd line AMBD at the same time. Standard operating procedures (SOPs) for sequencing MBT strains have been developed and a method of MBT sequencing has been established in the laboratory, when applied, it is possible to predict DR MBT.
