The aim of the work is to increase the effectiveness of surgical treatment of patients with multidrug-resistant pulmonary tuberculosis and pulmonary tuberculosis with widespread drug resistance. Research methods are clinical-instrumental, laboratory, cytological, microbiological, pathomorphological, statistical. It was determined that the use of a set of measures for the prevention of bronchial stump fistula reduces the level of postoperative complications by 8.4%. It has been proven that the early implementation of VATS pleurectomy made it possible to reduce intraoperative blood loss by 199.8 ml, reduce the average duration of surgical intervention by 1.5 times, reduce the length of hospital treatment by 9.9 days, and increase the effectiveness of treatment by 22.2 %. It was determined that the use of collapsosurgical staged surgical interventions made it possible to increase the effectiveness of treatment by 36.9%, shorten the period of inpatient treatment by 58.5 days, and reduce postoperative mortality by 12.3%. It was established that the use of a set of VATS measures makes it possible to increase the effectiveness of treatment by 7.5% and reduce the level of postoperative complications by 13.6%. The technology for the use of pulmonectomy and pleuropulmonectomy in the treatment of patients with pulmonary tuberculosis with multiple / extensive drug resistance with the use of methods of additional strengthening of the bronchial stump and prevention of pleural empyema was developed, the use of which made it possible to achieve the effectiveness of treatment in 86.4% of patients.
