Aim – to improve the efficiency of morphological diagnosis of pulmonary nodules by work out a rational algorithm for morphological examination. Methods – cytological, histological, histochemical, immunohistochemical, radiological, statistic. It was found, that the frequency of solitary pulmonary nodules among in patients of a highly specialized phthisiopulmonological institution amounts to 1,3 %. The average age of patients is (52,0 ± 0,7) years. The etiology of nodules was determined: malignant tumors – 37,7 %, inflammatory nodules – 32,6 %, benign tumors – 18,8 %, nodules of vascular origin – 4,6 %, others – 1,0 %, nature of the nodules not determined in 5,3 %. 3 variants for medical management of patients and their distribution were established: only diagnostics was in 52 (14,6 ± 1,7) %; diagnostics and surgical treatment – 45 (12,6 ± 1,6)%; diagnostic and therapeutic surgery – 259 (72,8 ± 2,4)% of cases. The overall diagnostic efficiency of cytological examination of pulmonary nodules was 65,7 %, and the histological – was 93,9%. Additional use of immunohistochemistry increases the effectiveness of diagnostics of malignant lung nodules by 16,1 %, which contributes to the choice of further treatment tactics for patients with this pathology. It was determined that in cases of necrotized foci of non-neoplastic origin the use of additional histochemical examination increases the efficiency of diagnosis by 26,5%; the overall efficiency of diagnostics in these cases is 85,3%. An algorithm for the morphological study of solitary focal lung lesions in hospital was developed, which increases the efficiency of histological diagnosis of cases of solitary foci of non-neoplastic origin based on surgical material up to 89,5%. Measures to optimize the work process to obtain the result of a histological examination more quick are proposed.Sphere of application – phthisiology, pulmonology, thoracic surgery, pathomorphology.
