Purpose of the work: to develop criteria for selection of surgical resection or radiofrequency ablation of metastatic liver lesions in colorectal cancer based on the selection of immune and biochemical predictors of the systemic inflammatory response. Methods of investigation and equipment: immunological, hematological, biochemical, clinical, morphological, statistical methods; light microscopes, flow cytofluorimeter, spectrophotometer, biochemical, hematological, and immune enzyme analyzers. Theoretical and practical results: the theoretical result consists in establishing the peculiarities of the immune state and changes in biochemical markers of inflammation in the development of an inflammatory response after radiofrequency ablation and surgical resection of liver metastases in patients with colorectal cancer. An increase in the product of the systemic inflammation response index and the diameter of the largest metastasis above 25.5 is recognized as a risk factor for a high intensity inflammatory response. The practical result is the development of a technology for predicting the degree of inflammatory response in patients after radiofrequency ablation of colorectal cancer liver metastases. Novelty: For the first time, a complex of indicators of the subpopulation composition of lymphocytes, which characterize the greater intensity of the inflammatory state, was determined. Prognostic factors were determined and an algorithm for predicting the development of a high-intensity inflammatory reaction during interventional treatment of metastatic liver lesions in patients with colorectal cancer was developed.
