The goal is to increase the effectiveness of treatment of patients with tuberculosis and non-specific lung diseases with concomitant pathology using objective indications for the use of valvular bronchoblockage. Methods and equipment - clinical, surgical, radiological, laboratory; computer tomography, video thoracoscopic complex, video fibrobronchoscopic complex, endobronchial bronchooccluder, bedside pleuroaspirator. An analysis of the diagnostic and treatment process was carried out in 107 patients with empyema of the pleura with bronchopleural complications of destructive lung diseases or their lesions in servicemen of the Armed Forces of Ukraine and civilians as a result of combat operations under martial law. It was established that 65,7 % of patients needed timely highly specialized medical care earlier. Recommendations were developed, the use of which gives reasons to reduce the duration and increase the effectiveness of treatment in the specified contingent. A methodology for determining the optimal stages and volumes of diagnostic and treatment measures in the specified contingents has been developed, which includes a set of measures for their examination, observation and treatment of patients at the stages of providing medical care, according to which contingents with the presence or a high risk of complication are determined treatment tactics at each stage of assistance, recommended criteria for referral to subsequent stages and appropriate diagnostic and treatment measures. The application of the method allows to ensure: personalized planning and treatment based on objective data, prevention of unnecessary surgical interventions or manipulations, reduction of the length of stay of patients in the hospital, on average, by 45,2 % and complete healing of 98,8 % of patients without the use of any what additional surgical interventions.
