Yanovsky National academy of medical sciences of Ukraine" popup.description2  Object of the study – 627 children with newly diagnosed tuberculosis (TB) from established and unknown contacts, statistical indices of the Ministry of Health of Ukraine regarding TB in children. The aim of the study is to improve the detection of tuberculosis infection and dispensary surveillance of children who have been in contact with patients with tuberculosis. Research methods: clinical-radiological, laboratory, statistical. It was found that the incidence of children in contact with a patient with (MDR+) in 2022 was 336 times higher than in the general population. It was established that 62.0% of the surveyed children were from unknown contacts, 81.3% were in contact with a (MDR+) patient. Children from contacts are more likely to have infiltrative pulmonary TB (24.1%) and children from unidentified contacts - bronchial TB (36.7%). Only in the group of contacts with XDR-TB patients the generalised forms of TB are detected, common clinical forms of TB (52.2%), MDR-TB (58.4%). In 63.7% of children with (MDR+), the results of resistance to anti-TB drugs are the same as those of the contact person, most often to rifampicin - 75.6%. A technology for managing children in contact with a TB patient regardless of bacterial excretion has been developed, which facilitates early detection of TB infection and reduces the risk of localized TB in children. A method for predicting the risk of developing localised TB in children in contact with a TB patient, which allows to increase the accuracy of prediction by 22.4% and to identify the contingents that need to be monitored by a paediatric phthisiologist to prevent the development of localised TB, and the technology for managing a child from contact with a TB patient without microbiologically confirmed bacterial excretion, which allows to increase the effectiveness of preventive treatment of a child by 17.2 % and prevent the spread of TB infection. were developed.
