Objective of the study: To reduce the risk of postoperative complications in oncological patients undergoing multi-organ surgeries and in cases of combat trauma by individualizing the tactics of intensive therapy. Research methods: clinical-instrumental, clinical, immunological, biochemical, statistical. Theoretical and practical results: The theoretical result involves establishing the specific gravity and structure of the Enteral Insufficiency Syndrome (EIS) in individuals after multi-organ surgical interventions and combat trauma. An assessment of the clinical-pathogenetic aspects of EIS in combat trauma and multi-organ surgical interventions is provided. It is demonstrated that the clinical-pathogenetic mechanisms of EIS development in combat trauma are significantly influenced by severe trauma of various origins with anatomical damage to abdominal organs: liver ruptures (7.2%), spleen (9%), kidney (9%), chest organs (13.5%), and gastrointestinal tract injuries (9%). The practical result involves identifying three periods of the clinical picture of EIS with specific symptoms. The main impairments of the absorptive function of the small intestine and their features for different periods of EIS are established. Diagnostic markers characterizing the state of severe endogenous intoxication, including indicators of medium molecular weight molecules, blood osmolarity, myoglobin, interleukin-1, and the index of peptide lymphocyte and polymorphonuclear leukocytes, are identified. Novelty: For the first time, the fundamental disruptions of the morpho-functional state of the small intestine, characteristic of different periods of Enteral Insufficiency Syndrome (EIS) in patients undergoing multi-organ surgeries and in cases of combat trauma, have been identified.
