Registration Date 05-01-2024 Organization State Institution "National Research Centre For Radiation Medicine of National Academy of Medical Sciences of Ukraine" popup.description2 The object is 789 children:140 patients with acute leukemia (АL);649- without oncohematological pathology (373-residents of radioactively contaminated areas (RСT),306- residents of conditionally "clean" areas).The purpose is to evaluate the hormonal and metabolic changes in the stromal-hemopoietic microenvironment in children with AL and living in the RСT after the accident at the Chernobyl nuclear power plant with an excess of iron in the body in order to increase the effectiveness of diagnostics, therapeutic and preventive measures (TPM). Research methods:cytological, biochemical,biophysical,cultural,immunological,dosimetric,statistical.Established for the first time hormonal and metabolic changes in the stromal-hemopoietic microenvironment and osteons in patients with AL and children with RCT with an excess of iron in the body,developed TMP.Children with AL after chemotherapy (CT) had increased level of serum iron (SI) and ferritin (SF),there were changes in bone formation.The amount of iron deposits in bone marrow sideroblast was correlated with SF,SI.Fatal cases were more often observed with SF level above 500 ng/ml.A direct correlation between the level of SF and cortisol,pituitary thyroid-stimulating hormone (TSH).12,9% of children without oncohematological pathology,residents of RCT,had the level of SI higher than 35,0 μmol/l;manifestations of an imbalance of amino acids in the urine,changes in the structure of collagen and thyroid function.Children with osteopenia and osteoporosis had bone fractures,hypermobility syndrome, increased levels of SI,alkaline phosphatase,TSH and calcium deficiency. Children's radiation doses did not affect osteon parameters and iron metabolism and amounted to (2,90±0,11) mSv.TPM corrected indicators of osteon metabolism:protein and mineral components of bones,iron metabolism, hormonal status.After TPM 81,3% of children with acute lymphoblastic leukemia and 78% -without oncohematological pathology had normalization of indicators.
