Research methods: determination of blood copeptin level using the immunoenzymatic assay, evaluation of prooxidant-antioxidant balance parameters by the colorometric method, investigation of genotypes and alleles of endothelial nitric oxide synthase gene polymorphism (eNOS, rs 1799983) and angiotensin-converting enzyme (ACE, rs4646994) gene polymorphism by the polymerase chain reaction method in real time. Based on the definition of associations between polymorphic variants of genes (ACE, eNOS) and the level of copeptin in blood serum, the state of prooxidant-antioxidant balance, cardio-hemodynamic and metabolic indicators, groups of increased risk of cardiovascular and renal disorders in type 2 diabetes were identified. It was established that the development of diabetic nephropathy (DN) is accompanied by a significant increase of copeptin concentration in blood plasma when compared to controls. The highest values of copeptin were found in the late stages of DN - in patients with albuminuria and impaired filtration function. It has been proven that patients with DN with high levels of copeptin in the blood serum have more significant violations of glucose metabolism indicators when compared to patients with normal values of this factor. The effect of pathogenetic combined cardio- and nephroprotective therapy on the level of copeptin in blood serum and the pro-oxidant/antioxidant balance in patients with DN - carriers of various genotypes of the studied polymorphisms was clarified. On the basis of the conducted research, personalized approaches to early diagnosis and treatment of cardiovascular and renal lesions in patients with type 2 diabetes were developed.
