It has been proven that the sensitivity compared to coronary angiography is 70.6% and the specificity is 41.17%, which is insufficient and requires invasive coronary interventions to clarify further medical care tactics for such patients. In 43.75% of patients, the determination of FFR leads to a change in the treatment strategy determined by angiography, in particular, in 25% of cases, interventional percutaneous intervention is required, and in 75% of patients, such stenoses are haemodynamically insignificant and require lifestyle modification and optimisation of pharmacotherapy. The study proved that intracoronary imaging using intravascular ultrasound allows to clarify the length of the lesion, reveals a higher frequency of atherosclerotic plaques with signs of severe fibrosis (25.0%, p<0.05), their surface (25.0%, p<0.05) and deep calcification (21.9%, p<0.05) compared with lipid plaques (12.5%) in the coronary arteries of the examined patients under stress caused by armed aggression, which requires a change in the tactics of percutaneous coronary interventions, in particular the use of high-pressure balloons and noncompliant balloons. The study shows that the use of IVUS allows optimising stent implantation, in particular, in 20.0% of cases, stent malposition is detected, which requires angioplasty using high-pressure balloons and noncompliant balloons.
