The purpose of the work is to investigate the intensity of expression of the CD28 receptor on CD3+ T-lymphocytes and costimulatory molecules of the B7 family (B7.1/CD80, B7.2/CD86) on a clone of tumor cells and monocytes; to study indicators of adaptive, innate immunity and clinical and hematological features in patients with AML in the first acute period before the start of treatment and after specific therapy. It was established that before the start of treatment, AML patients have a low level of intensity of expression by PC tumor cells with an immunophenotypic profile of CD34+, CD117+, CD33+ costimulatory molecules of the B7 family (B7.1/CD80, B7.2/CD86). It was shown that in patients with AML, before the start of treatment, the number of CD3+- T-lymphocytes with a low level of intensity of co-expression of the CD28+ ligand, which disrupts the intensity of immune reactions, is increased in the peripheral circulation. CD33+- monocytes with a high level of expression of costimulatory molecules CD80+-, CD86+- and T-lymphocytes with CD28+- ligand, which were found in the peripheral blood, are positive factors in controlling the duration of remission. It is shown that one of the reasons for the imbalance of antigen-specific antitumor immunity in patients with GML before the start of therapy is a decrease in the number of CD3+-, CD3+CD4+-, CD3+CD8+- T-lymphocytes in PC.
