Kavetsky Institute of Experimental Pathology, Oncology and Radiobiology National Academy of Sciences of Ukraine popup.description2 In the in silico system, based on systematization and bioinformatic analysis of public databases (Human Protein Atlas, UALCAN, TNMplot, TIMER2.0, Kaplan-Meier plotter), the features of the transcriptome and proteome of neoplasms were identified and the molecular-biological features of the microenvironment of breast cancer (BC) in young patients. Ex vivo on clinical material, for the first time as a result of a comprehensive study of the tumor microenvironment and factors of its regulation, 2 panels of biomarkers were created for predicting the course of the disease and the effectiveness of drug treatment of young breast cancer patients: a panel of 10 tumor-associated markers (COL1A1, COL3A1, COL12A1, PD-L1, MMP-9, miRNA-155, miRNA-182, miRNA-210, miRNA-199a, miRNA-145); a panel of 10 circulating markers (miRNA-182, miRNA-21, miRNA-27a, miRNA-29b, miRNA-34a, miRNA-145, miRNA-199a, IL-6, IL-10, and IL-17). It has been proven that the use of created panels of biomarkers allows to improve the immediate results of treatment of patients with BC at a young age due to an increase in the regression of neoplasms according to the WHO scale and clinical regression, which leads to an improvement in the quality and duration of their lives. Developed: protocol for the study of the predictive and prognostic value of tumor microenvironment markers; protocol for researching the expression profile of tumor-associated molecular biological and epigenetic markers of the reactive microenvironment for predicting the effectiveness of neoadjuvant polychemotherapy in young patients; a protocol for studying the profile of circulating regulatory molecules of the tumor microenvironment in the blood of young BC patients to predict the effectiveness of neoadjuvant polychemotherapy; 2 protocols for clinical validation and evaluation of the effectiveness of created panels of tumor-associated and circulating biomarkers for the purpose of individualizing the treatment of young BC patients.
