Purpose: to improve the results of treatment of patients with traumatic fractures of the vertebral bodies of the thoracic spine through clinical, radiological and experimental substantiation of rational treatment methods. Methods: clinical, radiographic, radiometric, computer-tomographic, magnetic-resonance-tomographic, physical biomechanical experimental modeling, mathematical biomechanical experimental modeling, statistical. Theoretical and practical results: the degree of destruction of the vertebra of the thoracic spine with the loss of elastic properties of the vertebral segment was revealed, and it was determined that stress equalization in the system "transpedicular structure - vertebral motor segment" is achieved under the conditions of eight-screw posterior transpedicular fixation with additional replacement of the destroyed vertebral segment with different methods of fracture treatment vertebra with an interbody titanium cage. Novelty: a new concept of treatment of a specific type of traumatic vertebral fracture of the thoracic spine, taking into account damage to the vertebral body and its posterior structures, has been developed. Effectiveness of implementation: The obtained new knowledge makes it possible to improve the results of treatment of patients with full recovery of working capacity in all cases of uncomplicated fractures or partial regression of mainly movement disorders in patients with incomplete impairment of spinal cord function types B, C, D, correction of post-traumatic kyphotic spinal deformity, restoration of sagittal diameter spinal canal thanks to the regeneration of the cancellous bone of the vertebral body.
