An algorithm of mandatory tests for assessing the risk of intravascular thrombus formation has been proposed, which includes measuring the concentrations of soluble fibrin, D-dimer, fibrinogen, and protein C. An increase in the procoagulant potential of blood plasma during pregnancy was established, particularly in the elevated levels of fibrinogen, soluble fibrin, and D-dimer. These changes were significantly more pronounced in pregnancies complicated by obesity. A decrease in platelet function was also noted, characterized by reduced speed and extent of aggregation in response to inducers like ADP, a thrombin-like enzyme, and a prothrombin activator. A new method for determining platelet aggregation has been developed, which allows for the evaluation of platelet response to thrombin while excluding the contribution of fibrin polymerization to clot formation. It was found that indicators of platelet activity, determined using this new method, correlate with markers of intravascular thrombus formation risk, particularly with D-dimer and fibrinogen levels, as well as body mass index. The method, which utilizes a prothrombin activator and fibrin polymerization inhibitor, proved to be more sensitive compared to the commonly used ADP-induced platelet aggregation assay.
