Registration Date 02-12-2024 Organization National Technical University "Kharkiv Polytechnic Institute" popup.description1 Development of methods for early diagnosis of complications in patients with the most common diseases of the cardiovascular system (hypertension, chronic coronary heart disease) and endocrine system (type 2 diabetes, hypothyroidism), who suffered from COVID-19, optimization of treatment and prevention measures based on laboratory assessment -instructive prognostic markers, which will significantly reduce the risk of developing and severity of complications of coronavirus disease popup.description2 Methods - molecular genetic typing of MTB by VNTR method at ETR A, B, C, D, E loci was performed in 235 TB patients, including patients with TB+COVID-19, and those with co-morbidity and multimorbidity. According to VNTR-typing, among the MTB cultures isolated from patients, VNTR profiles 42435 (30%), 22232 (23%) and 32333 (11%) had a large proportion. Taking into account the generally accepted criteria for determining the genotype of MTB, the Beijing type (VNTR profiles 42432 and 42435) was detected in 32 % of patients, the LA-M type (VNTR profiles 22232 and 22433) - in 26 %, the Harlem type (VNTR profile 32333) - in 11 %, and isolates of individual profiles (IAPs) - in 31 %. It was found that among patients with TB+COVID-19 and without co- and multimorbidity, patients with MTB strains of individual profiles predominated (45%), and among patients with TB with co- and multimorbidity - Beijing and LA-M (40% and 35%, respectively). The Beijing type was more common among people who had previously been in prison. The study found that acquired drug resistance to antimycobacterial drugs was more frequent in patients with LA-M and Beijing genotypes than in patients with Harlem and IAPs genotypes. Identification of the MBT genotype in patients with TB+COVID-19 allows predicting the course and outcome of the disease depending on the presence of co- and multimorbidity. The isolation of Harlem and IAPs types gives grounds to predict a favourable course of the disease, while the isolation of Beijing and LA-M types - a prolonged and less favourable course of the process. Genotype determination allows for timely adjustment of chemotherapy. Patients with Beijing and LA-M strains should receive more intensive antimycobacterial therapy with timely individual correction.
