The purpose of the study: is to determine the risk factors of local and distant metastasis and radiation toxicity during chemoradiation treatment of patients with cervical cancer in order to develop proposals for adjusting antitumor therapy and individualized patient support. Research methods and equipment: methods of non-parametric statistics, hidden knowledge search technology, special equipment was not used. Theoretical and practical results: the risk periods for distant recurrences and metastases have been determined; the most common distant and immediate radiological complications, risk factors, and methods of prevention have been identified; four risk factors for the development of distant cancer effects have been discovered. It has been substantiated that neoadjuvant polychemotherapy should be prescribed if the volume of the cervix at the start of treatment exceeds 60,000 mm³ and/or the level of tumor-specific marker SCC exceeds 9,1 ng/ml, and in the presence of tumor parametric infiltration on the right side or both sides. Adjuvant polychemotherapy should be prescribed if the ESR exceeds 16 mm/h and the SCC level exceeds 2,3 ng/ml at the time of patient discharge. Novelty: for the first time, a technology for individualized support of chemoradiotherapy for patients with locally advanced cervical cancer has been developed to improve treatment effectiveness by reducing the frequency of local recurrences, distant metastases, and treatment toxicity. Implementation effectiveness: the effect is an increase in overall three-year survival to 73%, five-year survival to 55,0%, a reduction in the frequency of local recurrences to 14%, and distant metastases to 17%.
