Research Goal: to reduce the risk of postoperative complications in oncological patients with multi-organ surgeries and combat injuries through the individualization of intensive therapy tactics. Research Methods and Equipment: clinical-instrumental, clinical, and immunological methods; flow cytometer FS-500, microplate photometer SUNRISE TECAN, cardiomonitor Hiaco, hematological analyzer XN-L550, ABL-520 device from "Radiometer Copenhagen," and Rapid Point analyzer from "Siemens Healthcare Diagnostics Ltd." Theoretical and Practical Results: it has been proven that the clinicopathogenetic mechanisms of sepsis development in combat injuries are largely caused by severe trauma of various origins with anatomical damage to abdominal organs: liver ruptures (7,2%), spleen (9%), kidneys (9%); thoracic organs (13,5%), injuries and wounds of the gastrointestinal tract (9%). It has been established that the development and progression of enteral insufficiency leads to a systemic inflammatory response with a threefold increase in IL-6 levels (median 17,3 pg/ml) in Stage III sepsis, along with progressive depletion of T-cell, phagocytic, and humoral components of the immune response. Novelty: for the first time, a two-level catheterization of the epidural space was proposed in the technology of intensive care of intestinal failure, which allows solving the problems of impaired motor and evacuation function of the intestine. Effectiveness of implementation: reduction of postanesthesia depression; reduction of the duration of the patient's stay on artificial lung ventilation after surgery; prevention of postoperative disorders of intestinal motor-evacuation function and reduction of the number of pharmaceuticals by 2,2 times.
