Purpose of the study: to develop a technology for a shortened course of chemoradiotherapy for head and neck cancer in extreme conditions, taking into account the mechanisms of tumor radioresistance. Research methods and equipment: radiological, morphological, clinical-instrumental, enzyme immunoassay, statistical; linear accelerator "Clinac-600C", planning system Eclipse 8.9, enzyme immunoassay analyzer "Immunochem-2100". Theoretical and practical results: an increased clinical efficacy of an accelerated course of radiotherapy for the treatment of squamous cell carcinoma of the head and neck was proven, with an increase in the proportion of complete responses to treatment by 8%, despite an increase in the frequency of radiation mucositis of grades II-III by 20%. The overall one-year survival rate is 89%, which corresponds to the indicators of classical irradiation (84%, p = 0.49). The study revealed increased levels of NF-kB in serum before the start of therapy, their dependence on the stage of the disease, lymph node involvement, as well as an increase in VEGF in 88% of cases. A positive clinical response is associated with stabilization or decrease in NF-kB levels (median -12%), while their increase (median +50%) correlates with the progression of the tumor process. Novelty: for the first time, the dependence of the clinical response to an accelerated course of radiotherapy on changes in the levels of NF-kB and VEGF has been established, which allows predicting the course of the disease and increasing the effectiveness of the treatment of squamous cell carcinoma of the head and neck. Effectiveness of implementation: reduction of costs for inpatient treatment, which averages 8000 UAH per patient per course of treatment. Field of application: radiation therapy, oncology.
