The aim − to improve the efficiency of molecular genetics (MG) methods of tuberculosis diagnosis (TB) by developing TB diagnosis algorithm and determining the drug resistance (DR) of MBT to anti-tuberculosis drugs (ATDs). Methods – genetic, bacteriological, statistical, equipment – MG systems and BACTEC phenotypic system. The method was developed for the determining of the DR MBT strains to H, Q and 2-nd line injectable drugs using GeneXpert MTB/XDR cartridges, which showed high validity of the test results in comparison with LPA testing DR MBT to H and ATDs 2-nd line and fTMS, namely sensitivity, specificity, predictive value of positive and negative results (practically 100 % coincidence of results). It is proposed to use this method in the practice of TB laboratories, and to implement the method of determining the drug profile of MBT strains into the TB diagnostic algorithm. An algorithm for diagnosis and monitoring of TB treatment with DR using rapid methods has been developed. It is proposed to use rapid MG methods as initial for all cases of TB disease, as well as for the purpose of accelerated determination of of DR MBT to ATDs.. Their use makes it possible to improve the quality of research and to shorten the time period for determining DR MBT to ATDs by 2 times. The developed SOPs for carrying out whole-genome sequencing of MBT strains allow to provide genomic characterization of MBT strains, as well as to investigate genetic variants associated with drug resistance to new and repurposed drugs and an established MBT sequencing method. Application of the method makes it possible to improve the quality of laboratory studies and shorten the time period for determining of the MBT MDR before accidents by a factor of 2.
