Yanovskyi National academy of medical sciences of Ukraine" popup.description1 To increase the effectiveness of surgical treatment of patients with multidrug-resistant pulmonary tuberculosis and pulmonary tuberculosis with extensively drug-resistance of Mycobacterium tuberculosis popup.description2  The object of the study is 479 patients with resistant forms of TB of the lungs and pleura who were operated on using resection, video-assisted or collapsosurgical methods. The purpose of the work is to increase the effectiveness of surgical treatment of patients with multidrug-resistant pulmonary tuberculosis and pulmonary tuberculosis with widespread drug resistance. Research methods are clinical-instrumental, laboratory, cytological, microbiological, pathomorphological, statistical. It was determined that the optimal terms of operative treatment of patients with MDR-TB and WDR-TB of the lungs are 4-6 months from the start of treatment. It has been proven that the use of collapsotherapeutic methods can increase the effectiveness of treatment by 8,6%. Carrying out lung resections (more than 3 segments) with corrective TP allows to increase the efficiency of the operation by 16.1 %. The technology of surgical treatment of patients with resistant pulmonary tuberculosis was developed, the use of which made it possible to improve the effectiveness of surgical treatment of patients by 12,9 %. It was determined that the use of a set of measures for the prevention of bronchial stump fistula reduces the level of postoperative complications by 8,4%. It has been proven that the early implementation of VATS PE with DC of the lung made it possible to increase the effectiveness of treatment by 22,2 %. It was determined that the use of staged collapsosurgical interventions made it possible to increase the effectiveness of treatment by 36,9 %. The developed technology makes it possible to achieve treatment efficiency in 86,4% of patients. It has been proven that with a morphologically determined low degree of inflammatory activity, MBT was detected only in the group of WDR-TB 17,0%.
