To develop an algorithm for managing a case of multidrug-resistant tuberculosis in a child, depending on the age and nature of resistance to anti-TB drugs
Object of study – 30 children with multidrug-resistant tuberculosis and drug-susceptible tuberculosis, statistical data of the Ministry of Health of Ukraine on TB in children. The aim of the study is to develop an algorithm for the management of multidrug-resistant tuberculosis in children, depending on the age and nature of resistance to anti-TB drugs. Research methods: clinical and radiological, laboratory, statistical. It was found that the incidence of children during the period of full-scale war increased by 1.4 times (up to 10.3 per 100 thousand people of the corresponding age), and adolescents by 1.6 times. The structure of clinical forms of TB has deteriorated due to the increase in the prevalence of MDR+ and destructive forms, drug-resistant TB. The incidence of contact children exceeds the incidence in the population by almost 395 times (in 2021 – 251 times), the incidence of contact adolescents – by 289 times (in 2021 – 124 times). It was determined that localized TB is most often detected in children aged 10-14 years (32.6%) and 5-9 years (26.3%). Pulmonary TB predominates in children of all age groups (72.2-82.6%), bacterial shedding is more common in children under one year old – 47.8% (in 2022 – 70.8%) and adolescents – 49.0% (in 2022 – 52.8%), in children of other ages – 20.3–26.6%. Among extrapulmonary TB, respiratory TB predominates (55.0–74.2%), followed by peripheral lymph node TB, and in children under one year old tuberculosis of the nervous system prevails. The technology of management of children at risk of MDR-TB infection and tuberculosis by timely and thorough examination using modern skin tests with recombinant proteins (ESAT-6 and SFP-10) has been proposed. This contributes to the early detection of TB infection, identification of contingents requiring observation by a pediatric phthisiologist and reducing the risk of local TB in children. You can enter a date or choose one by browsing through the calendar by months and years. By default, the interval starts from the oldest date (2018-01-23) and ends with the current date.
