Head Shevchuk Serhii V., д.мед.н. Registration Date 07-01-2025 Organization Vinnytsia National Pirogov Memorial Medical University popup.description1 To increase the diagnostic efficiency of structural and functional bone tissue disorders in patients with rheumatic diseases based on the evaluation of bone mineral density (osteocalcin, C-terminal propeptide of type I procollagen, pyridinoline, C-terminal telopeptide and type N collagen pepeptide, N procollagen type III) their condition, connection with the course of the disease and other metabolic risk factors (vitamin D, vitamin D receptor (VDR) gene polymorphism Fok I, hyperhomocysteinemia) and to create approaches for their pharmacological correction. popup.description2 Object of the study: male patients with AS, patients with SLE, practically healthy individuals. Purpose of the work: To increase the efficiency of diagnostics of violations of the structural and functional state of bone tissue in patients with rheumatic diseases based on the study of instrumental indicators of bone mineral density, as well as metabolic bone state, vitamin D status and VDR protein expression and their relationship with the course of the disease. Research methods: enzyme-linked immunosorbent assay (determination of interleukin-6, C-reactive protein, estradiol, osteocalcin, C-terminal propeptide of procollagen type I, pyridinoline, C-terminal telopeptide of collagen type I, N-terminal propeptide of procollagen type III), genetic analysis – polymorphism of the vitamin D gene. Instrumental research (radiography of the thoracic and lumbar spine, radiography of the hip joints, dual-energy X-ray absorptiometry, assessment of skeletal muscle tissue strength, assessment of skeletal muscle tissue functional capabilities. In the course of the study, the theoretical ideas regarding the role of metabolic factors in the development of progressive bone mass loss in patients with rheumatic diseases were deepened. The main factors that lead to the early development of a decrease in the mineral density of bone tissue are an increase in the total dose of glucocorticoids, the activity of the inflammatory process, vitamin D deficiency. On the other hand, no relationship was found between the traditional risk factors for the development of osteoporosis (age, duration of the disease, BMI) and state of bone tissue.  Product Description popup.authors Bezsmertnyi Yurii O.
