The presence of antibodies of the IgM class against the nucleocapsid antigen of the SARS-CoV-2 coronavirus was detected in 24.2% (16 patients), and antibodies of the IgG class against the nucleocapsid antigen of the SARS-CoV-2 coronavirus were detected in 95.5% (63 of 66 people) of the main groups This may indicate that due to the similarity of the pathophysiological mechanisms of COVID-19 and the progression of heart failure, the activity of the infectious process may be masked by symptoms of worsening heart failure, and such patients, remaining without proper treatment of COVID-19, may be at risk of developing complications of the infection. In patients of the main group, regardless of the presence of COVID-19 in the anamnesis, the activation of immunoinflammatory reactions (high level of biological markers of inflammation and ferritin, high metabolic activity of phagocytic cells against the background of exhaustion of their reserve capabilities) and an imbalance in the cytokine system were found: a significant increase in proinflammatory cytokines ( interleukin-6, tumor nexus factor) with a probable decrease in the content of anti-inflammatory interleukin-10. In patients with heart failure, regardless of its etiology, probably higher levels of the total pool of T lymphocytes (T3) and subpopulations of helpers (T4), killer-suppressor T-lymphocytes (T8), as well as natural killers were found. In patients with heart failure, regardless of its etiology, the presence of COVID-19 in the anamnesis, the presence of an autoimmune reaction with a significant increase in the levels of circulating immune complexes, autoantibodies to the myocardium, blood vessels and tissues of other organs was established.
