The most severe course of hypertension is observed in civilian patients with AH and with type 2 DM, in most of whom there is decompensation of diabetes and the development of heart, kidney and brain lesions is significantly accelerated. An important role in the progression of pathological hypertrophy and fibrosis of the myocardium in this category of patients is played by a decrease in the blood level of cardioprotective miRNA - 21-3r and an increase in the powerful component of the senescence secretome, insulin-like growth factor-binding protein 7. Uncontrolled hypertension and, especially, the decompensation of diabetes leads to the early development of albuminuria, which significantly accelerates kidney damage.
