Registration Date 21-01-2025 Organization Government Institution "L.T.Malaya Therapy National Institute of the National Academy of Medical Sciences of Ukraine" popup.description1 The aim is to substantiate the concept of the pathogenetic role of epigenetic factors of premature aging in martial law and postwar in the development of metabolic-associated diseases and to develop an algorithm for diagnosing and predicting accelerated aging popup.description2  To assess the effectiveness of the prediction and approach to the prevention of patients with metabolic-associated diseases in conditions of martial law, 252 people from the main and control groups were examined, including military personnel (41 people), 80 people underwent a partial examination (questionnaire). It was determined that screening for premature aging is effective for the early detection of patients who require correction of metabolic status. It was established that cardiometabolic diseases (arterial hypertension, dyslipidemia, fatty liver disease) are common among military personnel, which are associated with age, anthropometric status and the level of stress hormones. An algorithm for predicting the rate of aging and the development of metabolic-associated diseases in conditions of martial law and post-war conditions was developed based on determining the clinical significance of epigenetic markers of post-stress metabolic disorders. It has been proven that correction of metabolic disorders with metformin 1000 mg/day, rosuvastatin 20 mg/day and fisetin not only significantly reduced the calculated biological age, but also contributed to the increase in telomeres and normalization of SIRT1 (p< 0.05). A personalized approach to the diagnosis and prevention of metabolic disorders in wartime conditions has been developed, taking into account metabolic status, epigenetic markers and biological age, which allows for timely prevention of cardiometabolic diseases using non-drug and early drug intervention.
