Registration Date 22-01-2025 Organization State institution «Ukrainian center of maternity and childhood of the National academy of medical sciences of Ukraine» popup.description1 To investigate the role of birth defects and other pathological conditions in the structure of perinatal morbidity/mortality, taking into account the impact of COVID-19 and in the circumstances of Russian military aggression, to improve the methods of early diagnosis in order to optimize the management of pregnancy, labor and neonatal period popup.description2  Object of study: fetuses, newborns with normal development, congenital defects, chromosomal abnormalities, other pathology. Subject of study: results of prenatal, postnatal diagnostics of fetal and neonatal condition. Research methods: clinical and statistical, ultrasound, invasive procedures with cytogenetic analysis, immunological. Equipment: Voluson S10, MyLabTwise. The impact of COVID-19 infection on the pregnant woman, fetus and newborn was determined. Trends in patient referrals to the fetal medicine department during pandemic, full-scale invasion were analyzed according to the place of registration, pathology type, timing of diagnosis. Changes in the rate of various chromosomal abnormalities were identified, as well as increased proportion of older women with fetal Down syndrome. Late referrals were most often in lungs/chest, gastrointestinal tract, central nervous system malformations, tumors, and multiple anomalies. Immunophenotypic features of lymphocytes in blood of fetus and pregnant woman were analyzed. The almost complete absence of the NK-T like lymphocyte population was determined in the fetus, minimal levels of activated NK T cells expressing HLA-DR, which may be a sign of the integrity of their immune system against infectious agents. A high rate of premature birth, C-section for various congenital malformations were found, as well as small weight of neonates with heart defects, anterior abdominal wall, abdominal cavity, chest anomalies, a high rate of asphyxia (the highest level for lung/chest defects). Contribution of congenital defects was greater in the cases of antenatal and intranatal deaths, than in cases of neonatal deaths. Data on anatomical variants of malformations, typical changes during pregnancy, complications have been systematized, protocols/algorithms for prenatal examination have been developed, which help to establish a detailed diagnosis and prognosis, and choose patient’s management in circumstances of limited resources.
