The aim of the study is to improve the results of treatment of patients with dermal burns by developing a diagnostic algorithm for predicting the course of the wound process, improving and combining modern methods of surgical interventions, transfusion therapy regimens to reduce the risk of septic complications of the disease in thermal injury. Research methods: clinical, cytological, cytochemical, haematological, immunological. The developed programme for predicting the course of the wound process in patients with burns allows to monitor the indicators of toxic damage to peripheral blood phagocytes, indicators of functional activity of neutrophilic granulocytes, indicators of myeloperoxidase activity and the content of cells with apoptotic changes in patients with burns in the dynamics of burn disease. The application of the developed algorithm for surgical treatment of burns contributed to an increase in the content and optimisation of the functioning of monocytes and lymphocytes in the wound defect zone, which led to the optimisation of reparative processes in the burn wound zone. The use of the proposed treatment algorithm in complex surgical treatment leads to a decrease in the activity of reactions associated with oxidative stress, which reduces the manifestations of endothelial dysfunction, and promotes optimisation of interaction in the system of cascade cytokine interaction, reduces the activity of proinflammatory cytokines and regulates the development of an inflammatory reaction both in the area of thermal injury and in the body as a whole. A comprehensive assessment of the effectiveness of the developed programme shows a significant reduction in the incidence of local and systemic septic complications, and a reduction in the duration of inpatient treatment.
