The positive impact of integrating clinical pharmacy services into the healthcare system for improving the quality of medical care for the population of Ukraine was proven. It was established that providing professional information about medicines and medical products using modern communication systems is a priority among clinical pharmacy services, which can be considered as one of the areas of development of e-clinical pharmacy. Conceptual approaches to developing the Medication Management Program as a tool for monitoring the rationality of pharmacotherapy were scientifically substantiated. The importance of using special instruments (State Drug Formulary, STOPP/START criteria) to increase the effectiveness and safety of pharmacotherapy in older patients was proven. New approaches to increasing the value of a drug prescription through the systemic integration of professional communication, standardization of pharmaceutical care, and the medication behavior of a physician, pharmacist, and patient were proposed. Methodological approaches to assessing the patient's medication behavior were substantiated. The main determinants of patient non-adherence are identified, and the importance of influencing them to ensure high-quality pharmacotherapy in patients with chronic conditions and those who require lifelong treatment is proven. The insufficient level of awareness of pharmaceutical specialists regarding the problems of medication adherence is established, and ways to improve it are proposed. It is established that screening programs, based on modern scientific and innovative tools, are essential elements of the healthcare system; their implementation in clinical practice will contribute to effective monitoring of the medication process, prevention of the development of diseases and complications.
