The purpose of the research was to study the effectiveness of using VAC dressings after free skin flap surgery in patients with malignant skin tumors in comparison with conventional free skin flap surgery and skin-fascial flaps. According to the developed and standard methods, 137 thematic patients were treated: 53 patients in the main group, 39 patients in the first control group, 45 patients in the second control group. The applied modification of dermatome flap transplantation is quite effective compared to the routine scheme of surgical wound management (use of a negative pressure dressing during transplantation of free dermatome flaps). Complete flap engraftment in the first group was observed in 56.6 (30/53) % of cases, in the second group this result was achieved in 7.7 (3/39) % of patients, in the third group – 88.9 (40/45) %. In the first group, more than 50% graft engraftment was recorded in 84.9 (45/53) % of patients, the same indicator among patients in the second group was – 66.7 (26/39) %, in the third group – 93.3 (42/45) %. The proportion of cases of complete flap necrosis in the first group was 1.9 (1/53) %, and among patients in the second group – 12.8 (5/39) % and the third group – 0 (0/53) %.
