TUMORS OF CHIASMAL-SELLAR LOCATION, PITUITARY ADENOMAS, CRANIOPHARYNGIOMAS, MENINGIOMAS, OPTICAL COHERENCE TOMOGRAPHY, IMMUNO-HISTOCHEMICAL STUDY. Changes according to optical coherence tomography in patients with tumors of chiasmal-sellar localization were studied. The effectiveness of this method for establishing early indications for surgical treatment of patients with neoplasms of the chiasmal-sellar region has been proven. A classification of the severity of chiasmal syndrome was developed and implemented in clinical practice: the severity of chiasmal syndrome was assessed taking into account the visual acuity and field of vision (according to the total loss of light sensitivity - MD) in both eyes: mild chiasmal syndrome (visual acuity 1.0 in both eyes, MD up to -4 dB); moderate chiasmal syndrome (visual acuity above 0.1 in both eyes, MD from -4 dB to -12 dB); severe (visual acuity less than 0.1, at least in one eye, MD> -12 dB, at least in one eye). A classification of the spread of giant pituitary adenomas into the ventricular system has been developed and implemented into clinical practice, which allows determining the volume and necessity of plastic surgery of the postoperative defect at the preoperative stage. The classification is based on the data of MRI and MSCT studies (with intravenous contrast), which indicate the involvement of the ventricular system in giant pituitary adenomas. Type 0 - no interaction with the ventricles of the brain, 1 - adherence to the ventricles (3 and lateral) with their compression or deformation, 2 - spreading into the ventricular cavity, 3 - spreading into the ventricular cavity with blockage of the foramen of Monro and/or hydrocephalus.
