Trends in fertility, early neonatal mortality, neonatal mortality, stillbirth, causes of neonatal mortality, the incidence of extragenital pathology in pregnant women and perinatal risks of newborns in Ukraine up to 2025 were identified. The study assessed the staffing and organisational capacity of perinatal centres and maternity wards in Zhytomyr, Ivano-Frankivsk, and Chernihiv regions based on the performance indicators in the pre-war and war periods. A methodology was developed to determine the break-even point for perinatal care in a perinatal centre (supra-cluster level) and a maternity unit of a multidisciplinary hospital (cluster level) on the example of a healthcare facility in Zhytomyr Oblast. The staffing of departments providing care to pregnant women, women in labour, new mothers and newborns was developed, taking into account the optimal workload and regionalisation of perinatal care. The screening indicators for identifying pregnant women at risk of developing adverse antenatal events for the purpose of routing to perinatal centres for medical care during pregnancy and childbirth are determined. The study substantiates the improvement of the model of perinatal care in Ukraine in the context of the formation of a capable network of hospital districts, taking into account regionalisation.
