Aim: to determine the patterns of clinical, functional and structural-morphological signs of the bronchopulmonary system damage and the risks of complications developing in the remote post-accident period in persons exposed to ionizing radiation. Methods: generally accepted clinical and laboratory examination methods, pulmonary functional tests, spiral computed tomography of the chest, determination of sleep apnea-hypopnea using the SOMNOcheck micro device, statistical methods. The main clinical and functional patterns of COPD in clean-up workers of the Chornobyl NPP accident were determined: rapidly progressive course, slowly progressive course, PRISm. The rapidly progressive pattern of COPD was most frequently recorded in clean-up workers of the Chornobyl NPP accident and was characterized by a significant deterioration in intrathoracic pressure and lung diffusion capacity, a higher frequency of obstructive sleep apnea, cardiovascular diseases, and was significantly more common in clean-up workers of the “iodine period” (26.04‒15.05.1986 yy). Risk factors for the development of ventilation, and subsequently respiratory failure and a significant deterioration in the quality of life, an increase in socio-economic losses to society in clean-up workers of the Chornobyl NPP accident, COPD patients, are a rapid decrease in lung volume tests, an increase in lung reserve volumes and a decrease in lung diffusion capacity, a significant progression of emphysematous, fibrotic and sclerotic changes, a decrease in tolerance to physical exertion during dynamic observation. Based on the results of the research, 6 scientific papers were published, 1 report was made at an international scientific forum, and a new technology was presented.
