Registration Date 11-03-2025 Organization Bogomolets National Medical University popup.description1 The aim of the study is to improve the results of treatment of patients with purulent-necrotic wounds by optimizing healing processes and improving surgical tactics popup.description2  The dissertation provides a theoretical rationale and solution to the current problem, which consists in increasing the effectiveness of treatment of patients with chronic diabetic wounds by using applications of autologous plasminogen in complex surgical treatment.At the first stage of the dissertation, a study of angiogenesis factors and protease activity in diabetic wounds was conducted and comparison of these parameters in acute diabetic wounds. It was found that the expression of the central regulator of hypoxia-associated processes HIF-1α in diabetic wounds was increased 4.4 times compared to the values in acute wounds.In contrast, Western blots of VEGF from normal and ulcerated tissue samples show trace amounts of this growth factor in the diabetic wound. These indicators indicated severe hypoxia in wound tissues. At the same time, the expression of anti-angiogenesis regulators and VEGF shifts the angiogenic balance towards the ischemia state.The next step was to design a study to investigate whether topical application of autologous plasma-derived Pg could improve the healing of chronic foot ulcers in patients with type II diabetes. The obtained results of surgical treatment showed that applications of autologous Pg led to complete healing of a diabetic chronic foot ulcer.
