For the first time, scientific data have been obtained that violations of the longitudinal deformation of the LV occur in all layers of the myocardium (endocardial, middle and epicardial). It has been found for the first time that changes in the deformation properties of the LV myocardium in patients with GC begin with violations of the reservoir phase and are detected even with normal LV dimensions. Scientific data on the effect of GC on LV diastolic function parameters in patients have been expanded. It has been established that patients with GC and LV diastolic function that differs from normal have impaired LV reservoir function, which is reflected in a decrease in LV longitudinal strain. For the first time, an alternative parameter was proposed to patients with GC with an uncertain LVEF result according to the 7th algorithm of 2016 - the LV myocardial stiffness index with a threshold value of > 0.38 as a diagnostic criterion for LV diastolic dysfunction (LVDD). A significant negative effect of 24-hour hypertension and elevated pulse blood pressure on the LP reservoir function in patients with GC was revealed. It was established for the first time that the third stage of hypertension, elevated pulse blood pressure (PBP), pathological BP variability, and the presence of 24-hour hypertension contribute to a statistically significant increase in myocardial mechanical dispersion.
