In the group of patients receiving MSC injections, the safety of therapy and stimulation of granulation, reduction of microbial load without a negative impact on clinical healing rates were demonstrated. In both groups, the relative wound area gradually decreased during the observation period, while the wound healing rate in the control group and the MSC group did not differ. Transcriptomic analysis indicated early suppression of the immune response and activation of blood circulation and muscle contraction processes, while on day 7, modulation of chemokine pathways and regulation of immune mechanisms were observed. In vitro hUC-MSCs reduced the proliferation and pro-inflammatory activity of immune cells, promoting the formation of an anti-inflammatory phenotype of macrophages and changing T-lymphocyte subpopulations. The obtained data confirm the feasibility of using hUC-MSCs as a safe and promising adjuvant method in the complex treatment of complex blast defects of the lower extremities, which requires further clinical studies to evaluate and study the long-term effects, systemic and local effects of intrawound hUC-MSC injections on the immune system and reconstructive interventions.
