Study subject: 204 patients with acute myocarditis. Aim: To investigate the clinical course of myocarditis taking into account the impact of stress factors during martial law, to evaluate the effectiveness of differentiated treatment approaches for patients with different risk levels, and to identify predictors of complications over 24 months of follow-up. Results: In servicemen with myocarditis, predictors of persistent cardiac rhythm disturbances and a complicated disease course over 24 months of follow-up were identified. Among internally displaced persons, the greatest impact of stress factors related to martial law was found, which was associated with impaired heart rate regulation. Based on multivariable regression analysis, predictors of complications were established for patients of different risk categories. Approaches to personalized treatment of patients with myocarditis were improved. Factors were identified for the priority use of the sacubitril/valsartan combination in the treatment of heart failure in servicemen with severe myocarditis to reduce the volume of inflammatory and fibrotic involvement, restore left ventricular contractile function, and improve prognosis. The use of combined immunosuppressive therapy with azathioprine and glucocorticoids in patients with severe myocarditis in cases of ineffective glucocorticoid monotherapy was substantiated; this led to improved left ventricular systolic function and increased exercise tolerance. In servicemen, pregabalin use contributed to reduced anxiety levels and a lower frequency of supraventricular arrhythmias. The obtained results made it possible to develop practical recommendations for personalized management of patients with myocarditis under martial law conditions.
