Aim – to increase the effectiveness of surgical treatment of children with pulmonary and pleural tuberculosis. Methods – clinical, surgical, radiological, laboratory; computed tomography, videothoracoscopic complex, videofibrobronchoscopic complex, bedside pleuroaspirator. The study of the results of open operations indicates that the overall effectiveness of treatment is 94 (90.4 ± 2.9 %) cases. Indications for both open and minimally invasive interventions are the presence of morphological irreversible changes that lead to disability and prolongation of the disease. It was established that the advantages of open operations are satisfactory visibility, a small number of unresolved postoperative complications 2 (1.9 ± 1.3 %) cases, and the disadvantages are a longer duration of surgery (214.3 ± 20.2) minutes, a greater number of intraoperative 16 (15.4 ± 3.5 %) and postoperative 24 (23.1 ± 4.1 %) complications, 8 (7.7 ± 2.6 %) cases of disease recurrence. The advantages of VATS operations are a shorter duration of surgical intervention (185.8 ± 18.5) min, a small number of intraoperative 16 (14.8 ± 3.4 %) and postoperative 19 (17.6 ± 3.7 %) complications, 2 (1.9 ± 1.3 %) relapses of the disease. The disadvantages of VATS operations are conversion to thoracotomy, unsatisfactory visibility and a greater number of unresolved postoperative complications 5 (4.6 ± 2.0 %). The overall effectiveness of VATS operations is 101 (93.5 ± 2.4 %) cases, which is 3.1 % higher than the effectiveness of open operations. A method for choosing the tactics of surgical treatment of children with pulmonary and pleural tuberculosis has been developed, the use of which allows reducing the patient's stay in the hospital by 3.5 days and reducing the frequency of intraoperative and postoperative complications by 16.5 %.
