The aim of the study was to determine the effect of neoadjuvant androgen deprivation therapy on the perioperative course, pathological changes, as well as early and long-term oncological outcomes of radical prostatectomy, depending on the regimen and duration of its use. Research methods included clinical, laboratory, radiological, morphological, statistical, and analytical methods. A retrospective analysis was performed on 175 patients who underwent radical prostatectomy, of whom 84 received neoadjuvant androgen deprivation therapy, and 91 constituted the comparison group. They were stratified according to the duration of therapy (1-2 months, 3 months, more than 3 months) and its intensity (androgen deprivation therapy only and maximum androgen blockade). The data obtained were processed on a personal computer using IBM SPSS Statistics 22 and GraphPad Prism 10.4.1 software packages. The following statistical methods were used: Mann-Whitney and Kruskal-Wallis tests, χ² test, logistic regression, Kaplan-Meier curves, and Cox proportional hazards model. It was found that neoadjuvant androgen deprivation therapy does not worsen the course of the perioperative period. A tendency toward a decrease in the volume of intraoperative blood loss and a reduction in the duration of surgical intervention was revealed. In the group that received therapy for more than 3 months, a decrease in the volume of the prostate gland (33.4±7.2 cm³ vs. 42.1±8.5 cm³; p<0.001) and an increase in the frequency of pathological stage reduction (28.6% vs. 13.2%; p=0.018) were noted. The frequency of positive surgical margins was lower (21.4% vs. 34.1%; p=0.045), as was perineural invasion (36.9% vs. 52.7%; p=0.039). Product Description popup.authors Valerii S.
