MENOPAUSAL TRANSITION, GENITOURINARY DISORDERS, URINARY SYNDROME, FEMALE SEXUAL DYSFUNCTION. The study included 350 women aged 43 to 47 (46.3±2.21) in the final stage of menopausal transition, among whom 112 patients (32.0%) aged 42 to 46 years (45.1±0.4) with genitourinary disorders (GD), among women with GD 42 (37.5%) were patients from territories occupied by Russian troops and 70 (62.5%) from unoccupied territories. The aim of the study was to improve the methodology for diagnosing genitourinary disorders in women in the final stage of menopausal transition under martial law by determining the prerequisites for their occurrence, clinical and paraclinical characteristics, as well as psychosomatic, hormonal, immunological, biochemical, microbiological, ultrasonographic, and screening cytological studies. Research methods: clinical examination (including questionnaires), hormonal testing (estradiol), ultrasonography of the pelvic organs using Doppler imaging of the clitoral vessels, determination of nitric oxide levels in protein-free vaginal secretions, statistical analysis. GD was found in 32.0% of women aged 42 to 46 (45.1+0.4) at the final stage of menopausal transition, which is 3-5 years earlier than observed before the start of hostilities in Ukraine and was clinically confirmed by atrophic changes in the urogenital tract, taking into account the vaginal health index in the examined women: a higher degree of vulvovaginal atrophy was established in 8 (7.1%), manifested - in 16 (14.3%), moderate - in 68 (60.7%), mild - in 20 (17.9%) according to its manifestations, against the background of decreased levels of nitric oxide metabolites in vaginal secretions (≤30.0 nmol/mg protein) and hypoestrogenism (estradiol level below 50 pg/ml). An algorithm for early diagnosis of GD in premenopausal women has been developed, which allows for the identification of early signs of GD and adequate treatment.
