To determine the characteristics of inflammatory endotypes in patients with different phenotypes of chronic rhinosinusitis and to assess the impact of comorbid conditions and previous COVID-19 infection on the clinical course, immunobiochemical parameters, quality of life, and treatment effectiveness. Methods. Clinical, endoscopic, radiological, laboratory immunological and biochemical methods, as well as statistical analysis. Results. A comprehensive clinical and laboratory characterization of patients with different phenotypes of chronic rhinosinusitis was performed. The inflammatory endotype was identified as a key determinant of disease severity, frequency of exacerbations, and risk of recurrence. Patients with a type 2 inflammatory endotype demonstrated a higher symptom burden, poorer quality-of-life dynamics according to the SNOT-22 scale, and a greater tendency toward a recurrent disease course. A profile of the local immune response of polyp tissue was established, including prognostically significant cytokines, immunoglobulins, and proteolytic activity markers, enabling prediction of disease course and individualization of therapeutic strategies. The presence of comorbid conditions was associated with a more severe course of chronic rhinosinusitis, lower quality-of-life scores, and an increased risk of recurrence. Previous COVID-19 infection was accompanied by transient exacerbation of inflammatory activity and symptoms in the early post-COVID period. Scientific novelty. For the first time, the relationships between phenotypes and endotypes of chronic rhinosinusitis, comorbid conditions, and previous COVID-19 infection were systematically analyzed using integrated clinical and laboratory data. Practical significance. The findings support the feasibility of a personalized approach to the diagnosis and treatment of chronic rhinosinusitis and can be implemented in clinical, research, and educational practice.
